Population Health Management - Burnley neighbourhood case study
In Burnley East, the Primary Care Network team have taken a Population Health Management approach to develop tailored, personalised care for a group of people aged 50 and over living with frailty - combining health and communities together, looking at different aspects of people's life. Hear how they adopted a peer-to-peer model of engagement, learning more about the needs and aspirations of people as well as using data to identify people who might best benefit from early interventions to improve their longer term health and wellbeing.
