The Personalised Care Hub provides the health and care workforce across Lancashire and South Cumbria with education and support to continuously improve how they deliver person-centred and personalised care.
With specialist knowledge and skills around health coaching approaches; tools in tailoring services in relation to people’s knowledge, skill and confidence to manage their own health and wellbeing (known as Patient Activation); and Making Every Contact Count (MECC), we offer a range of training, as well as support for teams, neighbourhoods and ICP/MCPs to develop and implement their personalised care improvement plans.
During the immediate response to coronavirus (COVID-19), our face-to-face training has been suspended. However, we will use this page to collate a number of resources that we are developing that can help in our collective health and care system response to coronavirus (COVID-19). All resources are designed to provide you with quick and effective tools that you can use immediately to support people’s health and wellbeing, maximise self-management and reduce, as far as possible, any unnecessary demand on services.

Responding to Covid-19 by supporting people to self-manage their health and wellbeing
The Personalised Care Hub is supporting the workforce to maximise Personalisation, Activation and Supported self-management as part of the evolving response to Covid-19. As part of our programme of work, we are putting together a series of webinars to help teams accelerate and embed Personalised Care in their Covid-19 Recovery and Reset plans.
Health Coaching Skill Programme
The table below outlines the dates and timings of free health coaching training open to staff in February 2021. This training forms part of the workforce offer from the personalised care hub and is delivered by a group of health coach trainers who work across health and care in our local systems and organisations. Please sign up with the Eventbrite links provided. Once you sign up and get your ticket, the link will be sent to you two days prior to the training starting. The Health Coaching programme is a two session event with session one running for 3.5 hours, followed up with session two, a week or so later for a further 3.5 hours. You will receive pre-course information and a certificate. You must attend session one in order to attend session two.
- The programme is delivered using Microsoft Teams and covers health coaching approaches, several coaching conversation models and provides opportunities for coaching practice.
- It’s a helpful programme if you’re wanting to introduce a coaching style approach into your work and conversations and is a helpful way to enable you to empower a person to problem solve, set realistic and meaningful goals and support shared decision making.
- We ask all attendees to be online 10 minutes prior to start time, with microphones off, to check their IT is working and in case they need to resolve any issues with their IT dept.
- The course is participatory and there will be times you’ll be asked to contribute and even to try out your coaching skills.
