Living with and beyond cancer

The Living with and Beyond Cancer workstream will work collaboratively across the whole Alliance to ensure the people of Lancashire & South Cumbria are fully supported living with and beyond cancer and that their holistic needs are suitably met. We will work to ensure all eligible cancer patients have access to elements of the recovery package at the right time for the patient. The recovery package elements are:

  • Holistic Needs Assessment (HNA) and a written individualised care and support plan
  • A treatment summary completed at the end of every phase of acute treatment sent to the patient and their GP
  • GP Cancer Care Review (CCR) to discuss ongoing needs
  • Access to patient education and or relevant health & wellbeing support/events

  • Follow up care will be managed through risk stratification and will support patients to self- manage their care with confidence and offer the ability of robust re-entry pathways back into secondary care if required.


    Projects

    HNA project

    Project Lead: Jackie Brunton (Lead Cancer Nurse BTHT)

    • All eligible patients receiving a HNA by 2020.

    CCR project

    Project Lead: Dr Chrissie Hunt (GP Strategic Lead)

    • All eligible patients to be offered at least one CCR by 2020/21.

    HWB events project

    Project Lead: Scott Alker (Cancer Alliance)

    By 18/19 we expect 50% of eligible patients who have finished acute treatment to access HWB services which could be one or a combination of the following formats;

    • Risk stratified follow up workshop and or generic Health & Wellbeing Clinic
    • 1:1 support through the Macmillan information Support Centre
    • 1:1 support through the HWB Hub (funded through this bid)

    Prostate Project Lead : tbc

    • By 2018/19 80% of breast cancer patients will be on a self-management pathway
    • By the end of 2018/19 50% of colorectal patients will be on a self-management pathway
    • By the end of 2018/19 45% prostate patients will be on a self-management pathway

    E books Project

    Project Lead: Anne Tomlinson (Network Lead Cancer Nurse)

    • Implement eBooks for breast, colorectal, prostate and health and wellbeing eBook in all four trusts
